Dental Emergency Triage: What Needs Urgent Care

When something goes wrong with your teeth, the hardest question is often the simplest: do I need to be seen right away, or can this wait? Here is an honest guide — because not every dental problem is a true emergency.

A severe toothache that will not quit usually needs prompt attention — pain that lingers after hot or cold food, wakes you at night, or throbs on its own. That often points to an infected nerve or abscess, and infections do not resolve on their own. If the nerve is infected, root canal therapy may save the tooth, but only an exam and X-ray can confirm that.

A knocked-out tooth is the most time-sensitive emergency there is. Acting within the hour gives the best chance of saving it — but honestly, even with fast action a knocked-out tooth cannot always be reimplanted. It depends on how the tooth was handled and the condition of the root.

A broken or cracked tooth needs urgent care if it causes sharp pain, bleeding, or a large piece has broken off. A small chip with no pain can usually wait for a regular appointment.

Swelling in the face, jaw, or gums — especially with fever — can signal a spreading infection and should be seen quickly. Swelling near a back molar is sometimes a problematic wisdom tooth (see wisdom teeth removal in Scarborough). If swelling affects breathing, swallowing, or vision, go to the emergency room or call 911 — do not wait for a dental office.

Bleeding that will not stop, or a lost filling or crown leaving a sharp edge, also warrants a prompt visit. A painless lost crown can often wait a day or two — keep it safe and avoid chewing on that side. And when a tooth cannot be saved, dental implants are one long-term replacement option to discuss at a later consultation.

What To Do In A Dental Emergency Before You Arrive

The minutes between a dental injury and your appointment matter. Here is what dental associations such as the Ontario Dental Association and the Canadian Dental Association advise patients to do first:

Knocked-out tooth: Pick it up by the crown — the chewing surface — never by the root. If dirty, rinse briefly with water; do not scrub it or remove attached tissue. Try to gently place it back in the socket, or keep it in cold milk, and get to a dentist as quickly as you can.

Broken tooth: Rinse your mouth with warm water, save any broken pieces, and hold a cold compress against your cheek to keep swelling down.

Toothache: Rinse with warm water and gently floss around the tooth — sometimes the “emergency” is just food wedged where it should not be. A cold compress on the cheek can ease pain. Do not place aspirin directly on your gums; it can burn the tissue.

Swelling: Apply a cold compress in 15-minute intervals. If swelling spreads toward your eye or down your neck, or you have trouble breathing or swallowing, skip the dental office and go straight to the emergency room.

Bleeding: Apply gentle, steady pressure with clean gauze — or a damp tea bag — for 15 to 20 minutes.
